Please come dressed appropriately to evaluate and treat the main areas of concern. Workout attire or loose clothing such as pants that can be rolled up or removed such as button-up shirt with a T-shirt underneath. This allows for having access to evaluate shoulders for a neck problem or shorts or loose pants to allow access to evaluate knees and feet with a low back problem. Leggings are acceptable but can affect treatment for knee problems. Clothing to avoid includes delicate thin fabrics that do not stretch, dresses, tight undergarments or “shapewear”, or really thick denim.
Any detailed information you have from your past, including the actual x-ray images are helpful. MRI imaging reports are helpful. To obtain you would go to the location previously performed and request a Release of Records to have imaging provided. If recommended by Dr. Elhard you will be referred. Generally- xrays are preferred or 6-8 weeks of conservative treatment before insurance will cover MRI or CT scans.
Many imaging centers have direct pay pricing.
Medicare pertains to individuals generally over the age of 65 or on disability.
Medicare is very particular on what they will cover.
You must meet criteria for billing manipulative treatment:
An example of when there is NOT resolution is if your neck problem that feels better for a few days after a chiropractic adjustment and the pain returns.
3. There is neck or back pain or numbness, tingling, or radiating pain into the extremities.
In other words- Medicare does not cover maintenance-type care. When further clinical improvement cannot reasonably be expected from continuous ongoing care and the chiropractic treatment becomes supportive rather than corrective in nature, Medicare considers the treatment as maintenance therapy.
Medicare only covers spinal manipulation that is medically necessary for a new problem or a flare-up.
Non-Covered Medicare Services:
Many services are considered non-covered by Medicare. If you and your provider decide on these treatments you will be asked to sign an Advanced Beneficiary Notice (ABN) or a consent form acknowledging that you accept financial responsibility for non-covered services. These include:
Exams or Re-Exams, X Rays, Maintenance/Wellness care, Manual Therapy (Graston/ART/Muscle Work), Massage
Electrical Modalities (Electric Stim, Therapeutic Ultrasound), Home Care/Exercise Instruction, Dry Needling, Non-spine conditions (ie hip, shoulder, ankle, headache, TMJ, etc.).
Medicare Advantage Plans follow the same guidelines for covering spinal manipulation as indicated above for primary medicare.
Active Care Visits: You will be asked to fill out a Patient Summary Form in which you must be specific with a date and details as to when your pain increased. This then will need approval from Optum Health and typically may be 6 visits over 8 weeks. Your first visit is covered so you can be evaluated. It will be expected that you will complete a treatement plan.
Routine visits: Some policies include additional visits that do not need approval. You still may be asked to fill out a Patient Summary Form. These visits are called “Routine Visits. You must still have pain, numbness, or tingling but perhaps you only need one visit to get it “under control”. Generally one receives 10-12 visits per year to use as you wish. A treatment plan is not recommended.
Exams: Limited evaluations are generally covered but depends on the provider.
A chiropractic re-exam is warranted with a new injury or an exacerbation of an existing injury.
At Arne Wellness a re-exam will be necessary if more than 90 days has gone by since a previous chiropractic or natural medicine visit.

It is an energetic method of evaluating what supplements may be more appropriate than other supplements. It is a tool. It is not diagnostic. It is also not used with every visit. When there are so many options of natural herbs and nutrients to take it helps to prioritize. Energetic testing depends on electricality. We are all electrical beings because we get a shock when we walk across a rug. Every plant and nutrient is composed of the chemistry lab “periodic table chart” (ie. Na+, Ca+, Cl-, etc). I have so many supplements that I’ve accumulated! Can I just bring my bag or box of supplements from home so you can just see them and also muscle test? Yes. It is often helpful to have your supplements with you on your visit. But, most important is that you provide a detailed list of ALL supplements BEFORE your visit. Yes. You will need to go to your cabinet and look at what you have and write these out. This helps to give a heads up that we may need to prioritize what we discuss or schedule a longer visit.
